I'M A FAMILY IN NEED
If you're concerned about an emotional or behavioral problem, then you're in the right place.
I'M A LOCAL PROVIDER
We encourage all agencies, community organizations, and local groups to participate in our growing network of care.

Offering families in Camden County the help and care that they need.

WHAT DO WE DO?

10 Principles Of Wraparound

Camden County Partnership for Children’s ultimate objective is to develop, refine, and execute, the ISP based upon wraparound values and strength-based planning.

PerformCare

PerformCare is a national, full-service, behavioral health managed care organization. PerformCare connects young people and their families statewide to the care they need to be well.

The Family Support Organization

Camden County Partnership for Children works very closely with Family Partners to ensure our services are truly responsive to the needs of families.

Child and Family Team

Based on our extensive experience working with families since inception in 2004, the most important ingredient to success is the development of the Child and Family Team.

Confidentiality / HIPAA Guidelines

The HIPAA Security Rule defines standards, procedures and methods for the security of electronic Protected Health Information (ePHI). CCPFC is committed to the protection of client information at all times.

Individualized Services Plans

Camden County Partnership for Children is not a treatment or social service program. We help families find, create and develop options that will support them while they work on solutions to their problems.

Here are the 3 essential steps to getting started.

The New Jersey Children’s System of Care covers all 21 counties with 15 Care Management Organizations operating within a single county or across a region covering multiple counties. Each local system is comprised of the same elements.

PerformCare is the single point of access to obtain any behavioral health, substance abuse and developmental disability services available publicly throughout New Jersey for youth ages 5 – 21.

Call PerformCare at 1-877-652-7624 to access services, 24 hours a day, 7 days a week or visit them online at www.performcarenj.org

Exactly How Does This Work?

IMPORTANT: You must be referred to Caring Partners through PerformCare and the NJ System of Care.

1. Call PerformCare
(877) 652‑7624

A Care Coordinator will ask you some basic questions to help you figure out the best way to address your family's needs.

2. Complete a Needs Assessment

PerformCare may contact a clinician to aid you in determining if you require care management services for your child.

3. Referral to Our Services

If your child needs intensive care and support from a team of family & community partners, you will then be referred to us.
